Mental Health:  

The integrated care model offered by ABLE Community Services, LLC   

enhances quality of life and helps ensure that you achieve your desired outcomes in a measurable, maintainable and meaningful manner. ACS strives to treat the whole person and provide the highest level of care to all clients while translating new discoveries to educate future generations who will carry on this work. 

Some of our care offerings include: ongoing assessment of level of care and services required; coordination between mental health and primary care services; education, employment, and housing assistance; 24/7/365 crisis availability; substance use services; and specialty veterans services. 

Addiction Recovery:

 ABLE Community Services, LLC offers a multi-disciplinary and evidence-based treatment approach to addiction treatment in all of its programs. The primary objective of treatment is to improve the level of functioning of chemically dependent persons and to assist them in learning the attitudes, skills, abilities and information necessary for successful and fulfilling lives in recovery. 

Social Services:

ABLE Community Service, LLC offers several programs and resources for helping those we serve achieve well-being and independence through opportunities that protect, empower, respect choice and preserve dignity, especially for those who are least able to help themselves.

Our social programs address common issues that those throughout our community face. We work hand-in-hand with our clients’ treatment team to ensure all needs are fulfilled and an integrated approach is taken. 

Residential:

Stable and safe housing is the foundation for one’s well-being. Having your own living space yields a multitude of opportunities and stronger supports for children, youth, adults and families. 

ABLE Community Services, LLC provides a comprehensive range of supportive residential care that includes licensed Community Residences, Apartment Programs, and Supported Housing programs. Supportive housing is an effective strategy that combines affordable housing with coordinated services to help those struggling with chronic physical and mental health issues maintain stable housing and receive appropriate health care. Applying a person-centered approach and coordinated care model, participants are empowered to pursue their goals and reach their full potential. 

 Care Management:

 Our care management services offer a diverse and coordinated approach to improving health outcomes and quality of life. At ABLE Community Services, LLC, our approach is based on the belief that the needs of our clients are best met by creating an environment that helps them organize, make sense of, and navigate today’s complex health care system.

Our proactive, integrated model blends assessments with targeted management into a comprehensive program utilized by the care management team that includes: Assessments and Care Plans; Use of comprehensive and care specific assessments to assess medical, behavioral, social and functional needs; Evidence-driven goals and care plans; Reassessments and course corrections as determined by the client’s achievements and further needs; Home visits for those seriously at risk for poor health outcomes; Established and coordinated care plans for members being discharged from a hospital, nursing home or other facility.
